To assess the efficacy of sildenafil citrate (Viagra) in patients with erectile dysfunction (ED) who were treated with PDE5 inhibitors or placebo. The objective was to determine whether PDE5 inhibitors (Viagra) were efficacious in patients with ED that were treated with PDE5 inhibitors. A retrospective study was conducted with a total of 9,834 patients diagnosed with ED between November 1996 and June 2002. The primary endpoint of the study was the proportion of patients who were treated with PDE5 inhibitors or placebo. The secondary endpoints included the proportion of patients who were successful in achieving ED on PDE5 inhibitors or placebo after 6 months of therapy. The safety endpoints included the incidence of adverse events, number needed to treat (NNT), adverse drug reactions, and discontinuations. A total of 9,834 patients were included in the study. Of these, 1,664 patients (7.8%) experienced an NNT of at least 5, indicating an efficacy end point of PDE5 inhibitors (Viagra).
The protocol was approved by the institutional review board of the University of California, San Francisco (approval number: BIR/14). All patients had at least 1 ED diagnosis. In this retrospective study, the primary endpoint was the incidence of ED (defined as the first documented ED diagnosis in the first 12 months of treatment) during the first 6 months of treatment. The secondary endpoint was the incidence of ED in the entire population. Patients with ED treated with PDE5 inhibitors had a mean age of 67.9 years (range: 26 to 99 years) and the median duration of ED was 6 months (range: 2 to 24 months). In the control group, the median duration of ED was 6 months (range: 2 to 24 months).
Of these patients, 6,063 were treated with PDE5 inhibitors or placebo. The proportion of patients with ED on PDE5 inhibitors or placebo was greater in patients with ED treated with PDE5 inhibitors (22% vs. 6%, p<0.001) than in patients treated with placebo (18% vs. 7%, p<0.001).
Sildenafil citrate was efficacious in patients with ED that were treated with PDE5 inhibitors or placebo. It is well established that PDE5 inhibitors are efficacious in ED that were treated with placebo. However, when used in a treatment with a PDE5 inhibitor or a placebo, they are not usually efficacious. The incidence of adverse events in patients treated with PDE5 inhibitors or placebo was greater than in patients treated with placebo.
